Support Coordination
Participants are at the heart of everything we do. We engage with participants, their families and careers in the services and support we provide. We deliver NDIS Core and Capacity Building Supports based on participants goals and needs.
This support category strengthen a participant’s ability to design and then build their supports with an emphasis on linking to broader systems of support. Careway support services provides level one and level two support coordination.
Careway support services would:
- Help you understand your plan and supports
- Help you work towards your goals
- Connect you with services and support you need
- Arrange Allied Health Professionals to help you
- Connect you with your local community
- Help you interact with NDIS
- Help you with Plan Reviews
- Help you if things go wrong
Careway Support Services
Fees
The services and support you select determines the fees you have to pay. The fees are paid through your allocated NDIS funds. We collaborate with you to develop an individual agreement, one that is tailored to meet your needs. There are no hidden charges.
All areas of the support are well discussed before commencement and the payment process is straight forward with monthly statements.
